Your CHOC patient survey
This short video will describe the patient survey you may receive after your child’s visit or stay at CHOC.
Shortly after your visit, you may receive a survey asking about your CHOC experience.
The survey might be a phone call or an email.
You will be asked questions about how your child’s care team communicated with you and how they worked together to care for your child.
You will also be asked to rate your overall CHOC experience.
Your participation in this survey is very
