Patient- and Family-Centered Care
Patient and family-centered care is a partnership between our medical staff, physicians, associates and your family, working together to meet the needs of your child in the best way possible.
We encourage patients and families to be active members of the care team by asking questions, sharing knowledge and making decisions. We encourage you to ask questions regarding medications, tests and procedures. We are committed to making sure you that know how your child is doing and that you are involved in the plan of care. Our primary goal is to promote the health and well-being of patients and families.

Family Advisory Council (FAC)
Our mission is to enhance the delivery of the highest standard of comprehensive and compassionate healthcare for all patients and family members. Patients and families work in partnership with physicians and medical staff to facilitate the program and policy development that supports the goals of patient and family-centered care.
The Family Advisory Council consists of adult family members from diverse backgrounds and family types that accurately represent the broad spectrum of families served by CHOC. The FAC meets regularly to provide input on decisions, initiatives and discussions at CHOC.
FAC members bring their experience in partnering with physicians and medical staff to share their unique perspectives. The FAC is a way to:
- Seek and learn from the patient and family perspective.
- Promote a culture of patient- and family-centered care within the organization.
- Guide patient- and family-centered care implementation through collaboration to improve programs, services and policies.
- Enhance the delivery of high quality and safe care.
- Ensure the voice of the patient and family is considered in all matters.
A Family Advisor Council member is:
- An individual or family member who has experienced care in the hospital or primary/specialty care clinics and who has demonstrated an interest in partnering with the medical staff in the care of their family member.
- Someone who can offer constructive ideas for change and who has a special ability to help the medical staff better understand the patient and family perspective.
Online Community Insights Panel
This online panel offers an additional opportunity for CHOC families to share their insights. Patient caregivers can opt-in and be a part of the Community Insights Panel. When needed, our panel participants are sent a brief survey via email focusing on specific topics to enhance the delivery of care.
