Access Restrictions
Access to certain medical information is restricted for parents and legal guardians of adolescents aged 12, regardless of decision‑making capacity, per COPPA privacy law. If you want to get a copy of medical records, please complete the Request for Medical Records form.
To learn more about these regulations, see:
Patients aged 11 and under: Parents/legal guardians have complete access to the patient’s health information. Use this form to request portal access for your child ages 0-11.
Patients aged 12 to 17: Parents/legal guardians will have limited access to the patient’s health record. Limited access will include messaging, scheduling requests and bill pay. Parents/legal guardians will not have online access to their child’s clinical information at this time, but patients can request self-access at age 13. Use this form to request portal access for your teen ages 13-17.
Patients ages 13 and older with limited decision‑making capacity: If your child is age 13 or older and has limited decision‑making capacity, parents and legal guardians may be eligible for full access to the child’s MyCHOC account. To request access, complete this form and select “Yes” for limited decision‑making capacity. Our Health Information Management team will review the request with the care team and confirm the appropriate level of access.
Patients 18 and older: Patients may access their own information online. In cases where patients have limited decision-making capacity, parents/legal guardians may access the patient’s information online. Use this form to request a MyCHOC account.
MyCHOC Patient Portal Access
MyCHOC access depends on the patient’s age. Parents and legal guardians may have limited access for teens because California law protects certain adolescent health information.
