Before Completing this Form
If you have used CHOC Link or MyCHOC in the past, you already have an account and do not need to complete this form. If you’re trying to access MyCHOC using your CHOC Link login and are having trouble, you’ll need to connect your accounts. To do so, submit a support ticket here.
If you have forgotten your password, click the “Sign In” button on either the app or the MyCHOC website and click the “Forgot Password” link. Follow the instructions to reset your password.
Upon completing this form, you should receive an invitation to the MyCHOC portal via email within five business days. If you do not receive the email, please check your spam filter. If your child is 12-17 years old, you may have limited access to certain health information due to state laws.
