MyCHOC User Guide
Request copies of your child’s medical records.
Managing your child’s health just got easier. MyCHOC, our new, improved patient portal, will replace CHOC Link and become your personalized connection to CHOC. This page provides information and guidance on how to get started on MyCHOC and other tools available as part of CHOC’s Digital Experience Initiative.
Getting Started
1. Download the App or Access the Web App from your browser.
- Download the app by clicking the buttons below, or go to the Apple App Store/Google Play Store and search “MyCHOC”
- Tap “Install” to download to your device.
2. Create an Account: The MyCHOC app works with your CHOC Link Patient Portal account. If you already have a CHOC Link account, go to Step 3. If you do not have a CHOC Link Account:
- On the home screen, tap CREATE ACCOUNT
- Submit the request form
- A CHOC Link invitation will arrive in your email within 3 business days (check your spam filter if you did not receive your invitation
- Click the link in the email to complete your account setup
3. Sign In: if you already have a CHOC Link account
- From the home screen, tap SIGN IN WITH CHOC LINK ID
- Enter your email and password and tap SIGN IN
If you experience trouble signing in, follow the following troubleshooting guidelines:

Navigating the App
In this section, we’ll walk you through the five main navigation icons located at the bottom of the app screen. These icons are your key to accessing different features of the app quickly and easily. You’ll find everything you need, from your Home screen, where you can see important updates, to My Health, where your personal medical information is stored. The Get CHOC icon allows you to easily connect with us via phone, while Appointments helps you manage your visits. Lastly, the More icon gives you access to additional options and settings. Let’s explore each one in detail.
Home Page
The Home Page highlights key functions while showcasing new health blog content. Appointments, results, and messages are at the top of the home page. It is important to note that the Messages function will link out to CHOC Link. Once in CHOC Link, you can send medication refill messages from the medications button, appointment request messages from the appointments button, and general messages through the messages button. The toolbar across the bottom is accessible from any MyCHOC activity.
Get CHOC at the center of the toolbar is a click-to-dial button for CHOC’s 24-Hour Pediatric Nurse Advice Hotline
My Health
My Health provides medical record data from Cerner. Only the most recent data (<200 items) will load by default. If you want to pull a full health history, select the load more button to bring in additional records. Select one of the button options to jump into the desired data type. Within MyHealth, you can now use the “Learn More” hyperlinks to search definitions for test results, conditions, allergies, medications and immunizations. These are provided by Web MD, a trusted health library.
- Test Results: Lab and procedure results sorted by date
- Medications: All medications are sorted by date and categorized by current and past.
- *Allergies: Known allergies sorted by date.
- *Immunizations: All immunizations in CHOC’s record sorted by date. Please note this may not be a complete record of you child’s immunizations if they were performed at another organization.
- Health Conditions: Medical problem history sorted by date.
- Radiology: Imaging result reports sorted by date. Images are not included.
- Documents: Documents such as discharge instructions that can be viewed and printed.
*Non-sensitive data is available to proxies of adolescents in the 12-17 age range.
Appointments
The appointments tab details all upcoming and past appointments. Five days prior to the appointment, the “Time to Check In” option will appear. Opening up the appointment and selecting Check-In will launch the mobile check-in activity. You can also get directions and select call click to dial button to manage your appointment. If the appointment is a telehealth appointment, this will be indicated in the appointment location details. Telehealth appointments taking place on Zoom can be launched here.
In past appointments, select the down arrow to view/download relevant documents from the appointment such as discharge instructions.

Mobile Check-In
Receive email and text message reminders of upcoming appointments.
Patients and primary and care locations will receive reminders via email or text message directing them to confirm their appointment and check in online. MyCHOC users will see alert flags on their appointments letting them know check-in is available and can complete the registration process from inside the app or web experience. The MyCHOC app is not required to utilize this new service.

Check in and complete pre-registration activities, including identity verification, copay collection and pre-visit forms, from your own device (any smartphone, computer, or tablet) in the privacy and comfort of your home from the privacy and comfort of your home.

Getting Started
Begin Mobile Check-In by selecting the link sent by SMS and/or Email
You will initially receive an appointment confirmation and reminder 2 weeks prior to your appointment. 5 days prior to your appointment you will receive an SMS and/or email notifying you pre-registration is available. Select the link which will take you to the mobile check-in activity. After entering a multi-factor authentication code, begin by confirming the patient’s date of birth.
Contact Info, Insurance, Forms and Copay
Review your contact and insurance information, which will be filled in based on what is available in the EHR. Fill in the blanks as appropriate.
- Patient Demographics: Contact information, patient custody, language, race, ethnicity, SSN, PCP
- Billing Responsibility: Guarantor, relationship to patient, DOB, address, billing email address
- Guardianship
- Next of kin, legal guardian name, relationship, phone, language.
- Emergency Contact
- Primary, Secondary and Tertiary Insurance
- Insurance provider, member ID, upload insurance card photo.
- Secondary Insurance
- Tertiary Insurance
- Photo ID: Upload drivers license photo
- Forms
- Consent, Notice of Privacy Practices, Mutual Arbitration Agreement.
- Other forms may be required depending on if you are a new patient and the department you are being seen.

Billing
Our new billing solution features a redesigned look and feel for patient billing statements, while allowing patients to see all owed amounts consolidated on our digital portal to access and pay bills in one convenient place.

Using Patient Wallet
The CHOC Patient Wallet is your one-stop shop for all your CHOC bills. You can log in to your Patient Wallet at any time from your phone, tablet, or desktop to view and pay your bills, create payment plans, communicate directly with our billing department, and more!
Pay Your Bill in 3 Easy Steps
- Visit choc.patientwallet.org
- Enter the 9-digit SecureHealthCode and the total balance due from the upper right-hand corner of your bill. Click Find Bill.
- Before paying your bill, you will be prompted to create your PatientWallet account. Once complete, you can view affordable options and complete your payment
