Infection Prevention Information

Thank you for your continued work and dedication. As part of CHOC preparation for the Influenza season this fall, and in an effort to protect the health and safety of our staff, patients and families, CHOC will continue adhering to the guidelines used in prior seasons, including vaccination for all individuals who work and volunteer at CHOC. The seasonal flu vaccine will protect against 4 strains (quadrivalent vaccine) including the 2009 H1N1. Free, seasonal flu vaccines will be available in injection or intranasal spray and will be offered to everyone working and volunteering at CHOC. Please note that upon receipt of vaccination, individuals will have a pink sticker placed on their ID badges.

All volunteers must turn in a document (Volunteer Influenza Form, proof of immunization, or declination). Each of us plays a very important role in educating the public about preventing the spread of Influenza. Thank you for your support in ensuring a successful Influenza vaccination program, and maintaining a healthy environment in which to deliver and receive care. Volunteers at CHOC Hospital in Orange

IF YOU WILL BE RECEIVING THE FLU VACCINE AT CHOC:

Vaccines are now available at Associate Health (Monday-Friday 7:30am-4:00pm). Best time to visit Associate Health are Wednesdays, Thursdays and Fridays from 10 a.m. to 4 p.m. Be sure to have your CHOC ID badge with you for vaccination. Please take a copy of the Volunteer Influenza Form to be signed off by Associate Health. Associate Health will forward this form to Volunteer Services Department for tracking/recording. If you are younger than 18 years of age, you must complete the Parental Consent for Flu Form and have a parent with ID present when the vaccine is administered to you.

Vaccines are also available in the inpatient units and outpatient clinics. Be sure to have your CHOC ID badge with you for vaccination. Please obtain a copy of the Influenza Consent Form and turn it in to the Volunteer Services Department. If it is not received into our office, our records will show that you are not in compliance.

IF YOU WILL BE RECEIVING THE FLU VACCINE FROM AN OUTSIDE PROVIDER:

Please provide proof by submitting it to Volunteer Services Department to obtain the pink sticker to be placed on your ID badge. You can also scan/email your proof to volunteers@choc.org or fax it to (714) 509-4527. You can arrange the pick-up of your pink sticker with Josie Cervantes.

IF YOU CHOOSE NOT RECEIVE THE FLU VACCINE:

Those who decline the seasonal flu vaccine will be required to wear a surgical mask between December 1, 2014 and March 31, 2015 (may be extended due to continued circulating Influenza virus). Please print out the Influenza Declination Form, complete it, and submit it to the Volunteer Services Office. You can also scan/email your proof to volunteers@choc.org or fax it to (714) 509-4527. This applies to everyone who works or volunteers at CHOC (clinical and non-clinical areas).

Volunteers at CHOC at Mission Hospital

IF YOU WILL BE RECEIVING THE FLU VACCINE FROM AN OUTSIDE PROVIDER:

Vaccines are available at Mission Hospital’s Occupational Health (Monday-Friday 7:30am-8:00am and 2:00pm-3:00pm). Be sure to have your CHOC ID badge with you for vaccination. Please obtain a copy of the Influenza Consent Form and turn it in to the unit charge nurse (CHOC 5th Floor) to obtain the pink sticker to be placed on your badge. Please also send a copy to the Volunteer Services Department for tracking/recording. If you are younger than 18 years of age, you must complete the Parental Consent for Flu Form and have a parent with ID present when the vaccine is administered to you.

If you are receiving your vaccine from another provider please provide proof by submitting it to the unit charge nurse (CHOC 5th Floor) to obtain the pink sticker to be placed on your badge. Please also send a copy to the Volunteer Services Department for tracking/recording.

IF YOU WILL BE RECEIVING THE FLU VACCINE AT CCMH:

Vaccines are also available in the unit on a limited basis and only if the nurses are available. Patience is required as the nurses are attending to the patients. Be sure to have your CHOC ID badge with you for vaccination. Please obtain a copy of the Influenza Consent Form and turn it in to the unit charge nurse (CHOC 5th Floor) to obtain the pink sticker to be placed on your badge. Please aslo send a copy of your proof to the Volunteer Services Department for tracking/recording.
